Current medication list

Structured record of what the client is currently taking — medication name, dosage, frequency, prescribing physician, and start date. Always visible in the client chart.

Change history

When a client starts, stops, or adjusts a medication, the change is recorded with a date and reason. The history gives you a timeline of pharmaceutical changes alongside therapeutic progress.

Prescriber coordination

Record prescribing physician details for each medication. When you need to coordinate care or communicate about medication interactions with therapy, the contact information is in the record.
